There's a fine line between being a government-sanctioned killer working for the CIA, operating in foreign countries and being a government-mandated killer working as an MD, in the US under the threat of prison, loss of license and job.

I guess the Nuremberg Precedents (just following orders is not a defense) doesn't mean anything anymore.
